Device Integration
The ContinuousCare platform integrates with a number of consumer health devices. These integrations allow patients to send readings from these devices into the ContinuousCare platform.
Core integrations are supported on the ContinuousCare patient mobile application which is available on both iOS and android. Some other health devices are available on request and can be integrated into the custom-branded patient mobile apps (iOS and android).

iHealth Devices
The iHealth device integrations are based on their Server APIs and not through direct Bluetooth device integration. Patients have to enable a one-time access to their iHealth account and thereafter, all data read from iHealth devices can be synced to corresponding trackers in their patient record in the Virtual Practice.

iHealth Glucometers
With this integration, supported blood sugar readings from iHealth glucometers can be synced to the ContinuousCare platform via the iHealth cloud servers.
Following blood sugar reading types are currently supported:
- Fasting Glucose
- Post-Breakfast
- Pre-Lunch
- Post-Lunch
- Pre-Dinner
- Post Dinner
- Random
- Bedtime
iHealth Blood Pressure Monitoring Devices
With this integration support, blood pressure readings from iHealth BP monitoring devices can be synced to the ContinuousCare platform via iHealth cloud servers.
Supported readings:
- Blood Pressure Diastolic + Systolic
- Pulse Rate/Heart Rate
iHeath Pulse Oximeter
With this integration support, blood oxygen saturation and pulse/heart rate readings from iHealth pulse oximeters can be synced to the ContinuousCare platform via iHealth cloud servers.

- Oxygen saturation
- Pulse Rate/ Heart Rate
iHealth Thermometer
With this integration support, body temperature readings from iHealth thermometers can be synced to the ContinuousCare platform via iHealth cloud servers.

With this integration support, weight readings from iHealth weighing scales can be synced to the ContinuousCare platform via iHealth cloud servers.

Omron Blood Pressure Monitor (HEM-9200T/9210T)
Omron Blood Pressure Monitor (HEM-9200T/9210T) integration is a core integration that is available in the ContinuousCare patient mobile application and is available on request in the custom mobile apps as well. This is a direct device to app Bluetooth integration. Readings taken on the device can be synced with the patient mobile application.

- Systolic Blood Pressure / Diastolic Blood Pressure

LinkTop 6 in 1 Health Monitor
This is a 6-in-1 Health Monitor and the ContinuousCare platform currently supports 5 of these functions.

The LinkTop device can be integrated into the custom patient mobile apps on request. This is a direct device-to-app Bluetooth integration and readings taken on the device can be synced to the app.

Apple HealthKit Support for iOS Patient Application
Apples HealthKit is a central health repository, to which account holders can save various health tracker data using supported devices and its applications. Apple's HealthKit platform integration support is available with iOS ContinuousCare patient application. Users can sync various health tracker data from their Apple HealthKit account to corresponding trackers in their ContinuousCare account. This data can be shared with healthcare providers.
